This self-referral form is for parents/carers of a child who is over 13 but under 16 years of age or a young person over 13 but under 16 years of age who attends one of the secondary schools we work with.
If you have problem with filling out this referral, your school can do it for you. Speak to your teacher or the Designated Mental Health Lead in your school.
If you are experiencing other difficulties such as:
An existing mental health diagnosis. For example, clinical depression, post-traumatic stress disorder, obsessive compulsive disorder
High risk self-harm and currently experiencing suicidal thoughts/behaviours
Oppositional defiant behaviours and other conduct disorders
Severe ADHD and ADD
Moderate to severe learning difficulties
Child/young person in crisis or requiring out of hours support
Child/young person requiring substance misuse interventions
